Pregnancy is an important thing in a Sim's life, but right now the game isn't very realistic in that regard. Therefore, I decided to make a list of the best mods (in my humble opinion) that you should have in your game.

1. RELATIONSHIPS & PREGNANCY OVERHAUL (AKA WOOHOO WELLNESS)
Pregnancy preferences and varied reactions for both partners, Protection, surrogate mothers and artificial insemination, adoption, paternity test and drama, cheating module, teen pregnancy reactions, fertility surgeries, Woolly Transmissible Diseases, among many other features.
2. ULTRASOUND MOD
Go to the gynecologist and have an ultrasound to see your baby.
3. BABY SHOWER MOD
Now you can make the Baby Shower for your Pregnant sims!
4. BABIES FOR EVERYONE
Biologically, only women can get pregnant...but sometimes, even though we like realism, we also like a little "fantasy".This mod allows anyone to get anyone pregnant!
5. WONDERFULWHIMS
The WonderfulWhims mod created by TURBODRIVER is a more familiar strand of another mod by the same creator, called WickedWhims. One of the features of the WonderfulWhims mod is the inclusion of pregnancy with more realism: the menstrual cycle, abortion, giving up an unwanted baby for adoption, and donating unborn babies for scientific research have been added.
6. MC PREGNANCY MOD
Most people who use mods are for sure familiar with the MCCC (MC Command Center) mod. This is without a doubt the most complete tool for mod "lovers".One of your abilities is allowing you to control your Sims' pregnancy. With this mod you can, among other things, interrupt a pregnancy, change its duration, see how many babies a sim will have, choose the sex of the baby (or babies), choose how many babies you want to have and much more.
7. PREGNANCY AGING AND DEATH
What a lot of people don't do is that a pregnant Sim is practically immortal, no harm comes to them (doesn't age) while pregnant. For those who want a little more story progression and tragedy, there is a mod that does just that. This mod removes the immortality of a pregnant Sim, making their gameplay more realistic and interesting.
8. DISCOVER PREGNANCY
After your Sims try to have a baby, you usually know if the Sim is pregnant because of the Pregnant Sim animations, the autonomous vomit interaction, or the option to vomit that appears when clicking on the toilet. With this Mod, these and other animations will be disabled, thus adding more emotion and realism to your gameplay.
9. PREGNANCY IS FINE
It's perfectly normal for your Sim to not be thrilled with the pregnancy. Generally, when your Sim is pregnant, positive modifiers appear that last for days and this is not always realistic because pregnancy, like everything else in life, has its ups and downs. With this mod, Sims will not feel overly happy and excited about their pregnancy.
10. CYCLE - MESTRUATION AND FERTILITY
In the real world, not everyone can get pregnant. Some families try for years and fail. In The Sims 4, getting pregnant is very easy. With this mod, pregnancy becomes more realistic and challenging. Sims now have menstrual cycles that they must track to maximize their chances of having a baby. There are certain times on the calendar when they will have a greater chance to conceive and they should prioritize those days. The mod also adds menstruation headaches along with tampons and sanitary pads.
11. LABOR PUDDLES MOD
Your sims no longer need to feel generally happy about their pregnancy discovery (except in the case of child-hating Sims).With this mod, you have the ability to choose the mood your sims experience during pregnancy.
12. MISCARRIAGE MOD
If your Sims wake up with cramping, pain, or are very tired and hungry, it's best to go to a Hospital with them. When you get back, it's possible that everything went well, but there's a big chance that your Sim will return home with bad news and unfortunately have had a miscarriage.
13. QUADRUPLETS, QUINTUPLETS, SEXTUPLETS BABIES
Some players are not content with having twins!!!They want to have quadruplets or quintuplets. This mod allows you to have quadruplets, quintuplets or even sextuplets.
14. RISKY WOOHOO AND TRY FOR BABY CHANCES
In The Sims 4, there are only two ways a Sim can get pregnant: One is during alien abductions, and the other is through a Woohoo option that appears when your Sim has a high enough relationship with another Sim. In real life, the status of a relationship doesn't necessarily have anything to do with whether or not you have a baby. With the Risky Woohoo mod, your gameplay becomes a little more realistic.
15. REALISTIC CHILDBIRTH
This mod adds realistic options to childbirth, such as natural childbirth and planned cesarean section.
